What actually happened

The old grading systems, which assigned severity partly by whether you blacked out, were formally abandoned. The Zurich consensus statement in 2012, published in 2013, established the modern approach: symptom-based diagnosis, mandatory removal, graduated return-to-play, and the flat rule that an athlete with suspected concussion does not return the same day. The Amsterdam statement in 2023 refined it further, replacing strict rest with early light activity after the first 24 to 48 hours.

What forced the change was chronic traumatic encephalopathy. Bennet Omalu's 2005 paper on Mike Webster's brain, Ann McKee's work at Boston University, and the accumulating case series moved this from locker-room culture to a public health problem. Second impact syndrome, a catastrophic and sometimes fatal brain swelling from a second hit before the first has healed, gave urgency to the same-day rule.

The science is still moving and deserves honesty: CTE cannot yet be reliably diagnosed in the living, the dose-response relationship between hits and disease is not settled, and the autopsy series suffer from selection bias because families of symptomatic players donate brains. What is not in dispute is that "no blackout means no concussion" was wrong, and that a generation of athletes was sent back onto the field on that basis.
